OCR description
Henry County Hospital, the covered entity (CE), reported that its business associate (BA) experienced a hacking attack that compromised the protected health information (PHI) of 3,689 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses/conditions, and medications. The CE notified HHS and the BA notified affected individuals and the media. In response to the breach, The CE provided free credit monitoring and the CE and BA implemented additional security measures to protect PHI.
