OCR description
United Steelworkers Local 286, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 37,965 individuals. The PHI involved included names, Social Security numbers, dates of birth, drivers’ license and/or state identification numbers, passport numbers, and health insurance and other treatment information. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on email security.
