OCR description
The covered entity (CE), Ethan Health, reported that several employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 4,047 individuals. The PHI involved included names, dates of birth, Social Security numbers, physical addresses, claims information, lab results, medications, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
