OCR description
The covered entity (CE), Morgan Stanley, reported that an employee of its vendor inadvertently mailed the protected health information (PHI) of 535 individuals to the wrong addresses. The PHI involved included names, dates of births, addresses, Social Security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
