OCR description
The covered entity (CE), Upstream RollCo reported that multiple employees were the targets of an email phishing incident that affected the protected health information (PHI) of 748,678 individuals. The PHI involved included names, dates of birth, Social Security Numbers, physical addresses, claims information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
