OCR description
Delphi Drug & Alcohol Council, the covered entity (CE), reported that several employees were the subjects of an email phishing scheme that compromised the protected health information (PHI) of 6,288 individuals. The PHI involved included names, addresses, dates of birth, and drivers’ license and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data.
