OCR description
The covered entity (CE), Link Audiology, reported that an employee was the subject of an email phishing scheming that compromised the protected health information (PHI) of 2,608 individuals. The PHI involved included names, addresses, and claims and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional technical safeguards to better protect its PHI.
