OCR description
The covered entity (CE), Summit Eye & Optical, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 5,727 individuals. The PHI involved included diagnoses, and potentially Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided identity monitoring services and implemented additional administrative, technical, and security safeguards. Staff were also retrained in its requirements to protect and secure PHI.
