OCR description
University Urology reported that its business associate (BA) experienced a cybersecurity incident that compromised the protected health information (PHI) of 56,816 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and health insurance information. Following the discovery of the incident, University Urology notified HHS, affected individuals, and the media. University Urology mitigated the issue by strengthening its administrative and technical safeguards.
