OCR description
The covered entity (CE), Methodist Family Health, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 5,259 individuals. The PHI involved included names, addresses, dates of birth, clinical diagnoses, treatment, lab results, and medications. The CE notified HHS, the affected individuals, and the media. The CE also provided substitute notification to the individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
