OCR description
The covered entity (CE), United Healthcare Services - Single Affiliated Covered Entity, reported that it experienced a malware incident that affected the protected health information (PHI) of 26,561 individuals. The PHI involved included names, birthdates, addresses, claims information, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
