OCR description
The covered entity (CE), United Healthcare Services - Single Affiliated Covered Entity, reported that the protected health information (PHI) of 1,971 individuals was viewable by others via its member portal. The PHI involved included names, birthdates, claims and financial information, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards.
