OCR description
Smile Doctors of Arizona dba Grinz Orthodontics, the covered entity (CE), reported that its office was burglarized and five desktop computers were stolen. The protected health information (PHI) of 1,034 individuals was compromised. The PHI involved included names, dates of birth, gender, X-ray images, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE revised its policies and procedures, retrained its workforce members, and implemented additional technical safeguards.
