OCR description
The covered entity (CE), United Healthcare Services, reported that a third-party independent insurance agency fraudulently signed up 1,116 individuals to its Medicare health plan. The CE notified HHS and the affected individuals. The PHI involved included names and health insurance information. In its mitigation efforts, the CE provided complimentary credit monitoring services to affected individuals and implemented additional administrative safeguards to better protect PHI.
