OCR description
The covered entity (CE), Great Expressions Dental Centers, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 1,925,397 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and financial and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice of the breach on its website. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative, technical, and security safeguards. Staff were also retrained on the requirements to protect and secure PHI.
