OCR description
The covered entity (CE), Culbertson Memorial Hospital, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 69,765 individuals. The PHI involved included names, Social Security numbers, addresses, birthdates, claims and financial information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect PHI. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
