Asian Health Services

The covered entity (CE), Asian Health Services, reported that an employee was the victim of an email phishing scheme that affected the protected health information of approximately 6,270 individuals. The PHI involved included names, dates of birth, medical record numbers, genders, diagnoses codes, lab results, and other treatment information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided free credit monitoring and implemented additional administrative, technical, and security safeguards to better protect its PHI. Staff were retrained on email security.
