OCR description
The covered entity (CE), Humana, reported that an employee inadvertently placed the diagnoses of 9,372 individuals on the outside of mailing envelopes. The PHI involved included names, addresses, and diagnoses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE revised its policies and procedures, retrained its workforce, and sanctioned the employee responsible for the breach.
