OCR description
Core Recovery, the covered entity (CE), reported that a former employee hacked into its email system and compromised the protected health information (PHI) of 2,656 individuals. The PHI involved included names, diagnoses, lab results, medications, Social Security and drivers’ license numbers, addresses, birthdates, and claims and financial information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards.
