OCR description
The covered entity (CE), SEVA Medical Group, reported that an individual impermissibly accessed the protected health information (PHI) of 690 individuals. The PHI involved included names and email addresses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical safeguards, drafted new policies and procedures, and provided training to its workforce on HIPAA Privacy and Security Rules.
