OCR description
The covered entity (CE), Vincera Rehab, reported that it experienced a ransomware attack which affected the protected health information (PHI) of approximately 5,000 individuals. The PHI involved included names, addresses, phone numbers, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect PHI.
