OCR description
Community Research Foundation, the covered entity (CE) and business associate (BA), experienced a cyber-attack that compromised the protected health information (PHI) of 31,104 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, diagnoses, and other treatment information. The CE and BA notified HHS, affected individuals, the media, and posted substitute notice on its website. As a result of OCR’s investigation, Community Research Foundation implemented new technical safeguards, revised policies and procedures, and provided individuals with free credit monitoring.
