OCR description
The covered entity (CE), DMS Health Technologies, reported that it experienced a hacking attach that compromised the protected health information (PHI) of approximately 48,336 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, lab results, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. OCR provided the CE with technical assistance.
