OCR description
Henrietta Johnson Medical Center, the covered entity (CE), reported that its business associate (BA) experienced a hacking attack that compromised the protected health information (PHI) of 4,001 individuals. The PHI involved included names, dates of birth, ethnicity, medical record numbers, diagnosis codes, lab information, health insurance information, social security numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
