South Suburban Surgical Suites, LLC

The covered entity (CE), South Suburban Surgical Suites, reported that an employee was the victim of an email phishing incident that affected the protected health information (PHI) of 5,340 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, claims and financial information, diagnoses/conditions, medications, and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional technical safeguards.
