OCR description
The business associate (BA), Imagine360, reported that two of its vendors experienced a cyber-attack that compromised the protected health information (PHI) of 132,807 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, financial information, and diagnoses. The BA notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the BA offered identity protection services and the BA implemented additional technical safeguards.
