OCR description
The business associate (BA), edgeMED Healthcare, reported that it experienced a hacking incident that affected the protected health information (PHI) of 6,700 individuals. The PHI involved included names and treatment information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA enhanced its existing security protocols, added additional security measures, and implemented new technical safeguards.
