Public Health Management Corporation, the covered entity (CE), reported that it experienced a malware attack that compromised the protected health information (PHI) of 206,113 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, claims and financial information, lab results, medications, diagnoses, and additional treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. Workforce members were retrained.
