OCR description
The covered entity (CE), Miracle-Ear Flagship, reported that an employee of its business associate (BA) was the target of an email phishing scheme that compromised the protected health information (PHI) of approximately 3,560 individuals. The PHI involved included addresses and treatment information. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE terminated its relationship with the BA.
