OCR description
The business associate (BA), The Vitality Group, reported that a software application exposed the protected health information (PHI) of 20,387 individuals. The PHI involved included names, dates of birth, addresses, and Social Security numbers. The BA notified HHS and the affected individuals. In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect PHI.
