OCR description
The covered entity (CE), Konen & Associates dba Unified Pain Management, reported that multiple employees were the targets of an email phishing scheme that affected the protected health information (PHI) of 5,922 individuals. The affected PHI included names, addresses, dates of birth, drivers’ license and social security numbers, claims information, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided individuals free credit monitoring and implemented additional technical safeguards.
