OCR description
The covered entity (CE), Beacon Specialized Living, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 3,338 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE offered free credit monitoring services and implemented additional administrative and technical safeguards to better protect PHI.
