OCR description
The covered entity (CE), Hines Interests Limited Partnership, reported that its business associate (BA) experienced a cybersecurity attack that compromised the protected health information (PHI) of approximately 3,000 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE provided complimentary credit monitoring services to affected individuals.
