OCR description
Care N' Care Insurance Company, the covered entity (CE), reported that a software application used by its business associate (BA) exposed the protected health information (PHI) of 33,032 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and claims and financial information. The CE notified HHS, affected individuals, and the media.
