OCR description
Acupuncture and Integrative Solutions Incorporated, the covered entity (CE), reported that it experienced a cyber-attack that affected the protected health information (PHI) of 750 individuals. The PHI involved included names, phone numbers, e-mail addresses, and claims and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach the CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance regarding the HIPAA Rules.
