OCR description
The covered entity (CE), Associated Pathologists dba PathGroup Health Plan, reported that its business associate (BA) experienced a cyberattack that compromised the protected health information (PHI) of 2,031 individuals. The PHI involved included names, addresses, and birthdates. The CE notified HHS, affected individuals, and the media. The BA implemented additional administrative and technical safeguards.
