OCR description
The covered entity (CE), Stephen Harkins, DDS dba Harkins Pain & Sleep Management Group, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 6,411 individuals. The PHI involved included names and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical and administrative safeguards.
