OCR description
The covered entity (CE), Mindful Innovations, PLLC., reported that a former employee gained access to its computer system by using her previously provided credentials. The breach affected the protected health information (PHI) of 1,900 individuals. The PHI involved included names, dates of birth, Social Security numbers, addresses, claims and financial information, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
