OCR description
Pennsylvania Department of Human Services, the covered entity (CE), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 11,687 individuals. The PHI involved included names and case numbers. The CE notified HHS, impacted individuals, and the media. In its mitigation efforts, the CE implemented additional technical and security safeguards to better protect its sensitive data. OCR provided technical assistance regarding the HIPAA Rules.
