OCR description
The covered entity (CE), Paramedic Billing Services, reported that it experienced a cyber-attack that affected the protected health information (PHI) of approximately 93,610 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims and financial information, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards. OCR provided technical assistance.
