OCR description
The covered entity (CE), BlueCross BlueShield of Tennessee, reported that due to a computer error an employee inadvertently mailed the protected health information (PHI) of 2,688 individuals to the wrong recipients. The PHI involved included names, addresses, health insurance information, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE strengthened its administrative and technical safeguards.
