OCR description
The covered entity (CE), Baylor College of Medicine, reported that its business associate (BA) was the victim of a cybersecurity incident that affected the protected health information (PHI) of approximately 505 individuals. The PHI involved included names, dates of birth, Social Security numbers, and lab results. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional technical safeguards and provided complimentary credit monitoring services to affected individuals.
