OCR description
The covered entity (CE), HCA Healthcare, reported that it experienced a hacking incident that affected the protected health information (PHI) of11,270,000 individuals. The PHI involved included names, dates of birth, demographics and other information such as zip codes. The CE notified HHS and affected individuals. In response to the breach, the CE issued media, substitute and individual notice, and implemented additional administrative, technical, and security safeguards.
