OCR description
The Health Plan of West Virginia, the covered entity (CE), reported that its business associate (BA) experienced a cybersecurity incident that compromised the protected health information (PHI) of 1,292 individuals. The PHI involved included names, addresses, phone numbers, and health insurance and financial information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the BA implemented additional technical safeguards and provided complimentary credit monitoring services.
