OCR description
Grane Supply, the covered entity (CE), reported that an employee sent an email that contained the protected health information (PHI) of 798 individuals to an unauthorized individual. The PHI involved included names and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts staff were retrained on the requirements to protect and secure sensitive data.
