Plumbers Local 8 Health & Welfare Fund

The covered entity (CE), Plumbers Local 8 Health & Welfare Fund, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 4,790 individuals. The PHI involved included names, birthdates, addresses, email addresses, telephone numbers, claims and financial information, diagnoses/conditions, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
