OCR description
The covered entity (CE), Advanced Care Partners, reported that an employee sent an email containing the protected health information (PHI) of 518 individuals to unauthorized individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and medications. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on the requirement to protect and secure sensitive data.
