OCR description
The business associate (BA), Cognizant Technologies Solutions, reported that it experienced a cybersecurity incident that compromised the protected health information (PHI) of 7,313 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and claims and financial information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect PHI.
