OCR description
The covered entity (CE), the City of Dallas, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 5,058 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and treatment and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards.
