OCR description
The business associate (BA), Radius Global Solutions, reported that a software application exposed the protected health information (PHI) of 632,204 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, claims information, and other treatment information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA implemented additional administrative, technical, and security safeguards to better protect PHI.
